Data controller

The data controller is the clinic and physician whose contact details appear on this site. The registered name, tax details and service address will be added to this section before publication.

Data processed

The appointment form collects only your name, phone number, an optional email address, the subject you selected and any free-text message you write.

The form is not intended to collect health data. You are asked to share details of your medical condition during your consultation, not through the form.

Purpose and legal basis

Your data is processed solely to assess your appointment request and to contact you, on the basis of your explicit consent. The form cannot be submitted without ticking the consent box.

Transfers

Form contents are delivered to the clinic’s email address through the email service provider used for transmission. Data is not otherwise shared with third parties, used for marketing, or sold.

Retention

Appointment requests are retained for as long as needed to handle the request and are then deleted. The exact retention period will be determined and stated here before publication.

Your rights

Under Article 11 of the KVKK you have the right to access your data, request its correction or deletion, and withdraw your consent. You can make such requests using the details on the contact page.
